Our Introduction

Make a Real Change In Someone's Life

The Guided Path Foundation is a 501C3 private not-for-profit that works primarily with those in need in Tampa-Hillsborough County Florida. We provide access to healthcare services, support services, and educational opportunities with the intention of improving the health outcomes within this population.

We offer a wide array of services, including HIV testing and linkage to treatment and care, educational and prevention workshops, mental health counseling, linkage to support services, hot meals, a clothing closet, transportation, and career readiness training to ensure that they can better thrive on an individual, interpersonal, and professional setting.

SNAP Food Stamps Benefit Application Assistance

HIV Education and Prevention

HCHCP (Hillsborough Country Health Plan)

Assistance with completing application for healthcare.
Hillsborough Health Care is a comprehensive managed care program for Hillsborough County residents with limited income and assets who do not qualify for other health care coverage, including Medicare and Medicaid. The program is funded by a special sales tax and administered by Hillsborough County.

SSI/SSDI Assistance with Application

Outreach, access, assistance with SSI/SSDI application.
Assist children and adults in applying for Social Security disability benefits, including those returning to their communities from homelessness, jails, and hospitals.

Sight Program

We have the application for people that don't have insurance and can't afford glasses or eye exam. They can complete our financial application.

Notarization will be $10.00 each.
Current Valid state or government Photo ID required for each signer. Name on ID must be exactly same as name on document.

Our Mission

Guided Path mission is to help individuals find a path to sustainable self-sufficiency by creating a multi-function service safe haven that provides our community path to begin their journey back to healthy and independence.

Our Vision

Identify each individual's respective physical, mental, social, and, or economic needs and develop a service plan according to their specific needs and pave the road to end homeless.

Eligibile Applicants must:
  • Be a resident of Hillsborough County
  • Be a U.S. citizen or a documented legal resident of the U.S.
  • Have income that is at or below 175 percent of the federal poverty guidelines
    • Meet asset allowance guidelines
  • Not be eligible for any other health care coverage, including Medicare and Medicaid
    • Medically Needy Program participants may be eligible for the Hillsborough County Health Care Plan
  • Provide documentation of Hillsborough County residency using any of the documents ID, SS card, proof of income etc.

    There is no cost to eligible members for services covered by the Hillsborough Health Care Plan.
